Advanced practice nurses use population health competencies to identify population needs by looking beyond the needs of individual patients and recognize health concerns affecting larger groups of people. They identify patterns such as high rates of chronic disease, limited access to healthcare, or other health disparities within the populations they serve. Advanced nurses are well versed in health promotion, disease prevention and patient education (Mackavey et al., 2025). They also consider social determinants of health, such as income, education, transportation, and access to healthy food, because these factors can have a major impact on a person’s overall health.
Advanced practice nurses work with physicians, pharmacists, nurses, social workers, public health agencies, and community organizations to provide coordinated, patient-centered care. Since a lot of the their focus is on preventive care and health promotion, this enables them to collaborate with local communities food banks, transportation services, and faith-based organizations to address barriers that affect health, including food insecurity, limited access to healthcare, and inadequate housing.
Some challenges advanced practice nurses face when integrating population health competencies into their practice are things like appointments times, staffing shortages, the population have limited access to resources and not having health insurance (Mackavey et al., 2025). Most of health care is focused on treating acute illness instead of preventions. Having things like Telehealth, mobile clinics, good EHR to recognize high risk patients, and strengthening interdisciplinary teams can help with the issues.
Organizations and healthcare systems can support advanced practice nurses in implementing population health strategies and initiatives by providing continuing education, invest in data analytics and care coordination technology. They can also grant full practice authority, allowing advanced practice nurses into executive leadership and to practice at the top of their license to help improve outcomes for both individual patients and entire populations.

Population health refers to an interdisciplinary approach to care that focuses on collaborative practice and innovative partnerships with community stakeholders to improve quality of life. Stakeholders have vested interests in their communities and include such entities as area businesses, school systems, public health agencies, to name a few.
