A.Reflect on your learning experience in this course. Which concepts stood out to you and made an impact?  How do you envision using these concepts in your future nursing practice as a masterâ€s prepared nurse practitioner?
One takeaway I found really valuable was Just Culture versus Blame Culture from Week 3. While I understood the need to report safety incidents before this class, I have gained a deeper appreciation for the significance of our reaction to errors, not just the errors themselves. When working in a blame culture, staff can become frightened, silent, and not wanting to report near misses or errors. Just Culture addresses this by creating a healthy balance between holding people accountable and growing your system. Just culture does not excuse unsafe practice but evaluates if the mistake was due to human error, risky behavior, or reckless behavior. Barkell and Snyder (2021) describe how Just Culture promotes error management, leadership engagement, and systems change. All of these are key when thinking about patient safety.
Another section that really resonated with me was the ethical implication of patient satisfaction surveys being tied to hospital reimbursement in Week 6. While I understand the importance of patient experience, I also know how this creates unnecessary stress for providers. Patients can demand antibiotics, opiods, unnecessary imaging, etc. When scores/reimbursement become the main focus, we as providers may feel we need to keep the patient happy instead of having a tough conversation that may be clinically appropriate. As I move forward as an AGACNP, I hope to provide compassionate, respectful care while providing safe and ethically appropriate care.

The assignments for this course allowed me to reflect on how quality and safety can be compromised by communication, culture, access, health literacy, and system barriers. As an AGACNP, I will be providing care to many patients of diverse backgrounds and I cannot presume that each patient has access to the same resources or have the same level of understanding/support. Using reflective practice will allow me to look at what went right, what could have been done differently, and what I can do better the next time. Cultural humility will allow me to listen more attentively to my patients and take into consideration the patientâ€s values, family, language needs, and social circumstances when planning their care.

I learned from this course that patient safety is more than simply adhering to policies. Patient safety also includes speaking up, reporting near misses, engaging in quality improvement activities, and utilizing data to drive improvement in outcomes. As an AGACNP, there are many ways I can help create a culture of patient safety. I can help ensure safe patient handoffs, medication safety, infection prevention, and clear discharge planning. Additionally, as an AGACNP, I can foster a team approach where staff feel comfortable reporting mistakes without fear of punishment, so we can learn from our mistakes and prevent patient harm.

One concept I gained from this course is that patient safety is not just an individual nurseâ€s responsibility. It is also a system-level responsibility. I learned that mistakes, near misses, poor communication, and weak processes should be examined with care, not just blamed on one person. As a future masterâ€s-prepared nurse practitioner, I will apply this concept by asking why this safety incident occurred. What system factors led to it, and what changes can prevent it from happening again.
A second idea I learned was cultural humility. This class made me realize that cultural humility is a continuous process of reflection, listening, and learning from patients. I cannot presume to know a patientâ€s values because of race, religion, language, or background. The program outcomes are supported by the course activities and assignments because the course encourages reflective practice and respect for cultural diversity. In practice, this will make me more attuned to patients, reduce assumptions, and include patient beliefs and preferences in care decisions.
The AACN sub-competency that resonates with me most is 5.2i: Design evidence-based interventions to mitigate risk. The study of quality improvement and patient safety in the course assignments helped me identify risks and create practical solutions. For instance, if medication errors increase in a clinic, I would not just report the issue but investigate what is going wrong. I would assess the process, determine where the risk occurs, and support evidence-based interventions like medication reconciliation, staff education, and improved handoff tools. Staff training, teamwork, open communication, and shared learning were seen to improve safety culture by Garay et al. (2023). A question to ponder is what patient safety issue you think will be most challenging in your future NP role, and what will be your first evidence-based intervention?
