Follow these guidelines when completing each component of the discussion. Contact your course faculty if you have questions.
General Instructions
The purpose of this discussion is to examine an assigned disease process and determine which disease process explored in the discussion Bayani, the client, is experiencing. Follow the instructions below and your faculty member will reveal which disease process Bayani is experiencing at the end of the discussion week.
Step 1: Review the following client scenario:
Bayani, a 62-year-old male client, presents to the NP with abdominal pain with his wife. Bayani is mildly confused, and according to his wife, he is normally fully oriented. His abdomen is soft and non-distended, and his wife states he has been urinating and drinking more water than usual. She also states he has had foul-smelling urine and is concerned he also has a urinary tract infection.
Step 2: Review your assigned disease process from the chart below. Your assigned disease process is based on the first letter of your last name.
Last Name
Disease Process
A – H
Condition: Peptic Ulcer Disease (PUD)
I – P
Condition: Cirrhosis
Q – Z
Condition: Type 2 Diabetes
Step 3: Review the client case scenario and answer the discussion prompts below with explanation and detail, providing complete references for all citations.
Step 4: Reply to a peer with a different assigned disease process.
Include the following sections:
1. Application of Course Knowledge: Answer all questions/criteria with explanations and detail.

Discuss the underlying pathophysiological mechanisms of your assigned disease process. Which clinical manifestations observed in Bayaniâ€s case could be explained by the pathophysiological mechanisms?
Analyze Bayaniâ€s clinical manifestations in the context of your assigned disease process. Do these findings support a diagnosis of your assigned disease process? Why or why not?
Identify and justify the diagnostic tests (including labs, imaging, or other diagnostic tests) that would be most appropriate for investigating a diagnosis of your assigned disease process in Bayani. What could the results of these tests look like in your assigned disease process?
Compare and contrast your response with a peer assigned a different condition. Does their condition fit Bayaniâ€s case? Why or why not?

2. Integration of Evidence: Integrate relevant scholarly sources as defined by program expectations:

Cite a scholarly source in the initial post.
Cite a scholarly source in one faculty response post.
Cite a scholarly source in one peer post.
Accurately analyze, synthesize, and/or apply principles from evidence with no more than one short quote (15 words or less) for the week.
Include a minimum of two different scholarly sources per week. Cite all references and provide references for all citations.

3. Engagement in Meaningful Dialogue: Engage peers and faculty by asking questions and offering new insights, applications, perspectives, information, or implications for practice.

Peer Response: Respond to at least one peer on a topic other than the initially assigned topic.
Faculty Response: Respond to at least one faculty post.
Communicate using respectful, collegial language and terminology appropriate to advanced nursing practice.

4. Professionalism in Communication: Communicate with minimal errors in English grammar, spelling, syntax, and punctuation.
5. Reference Citation: Use current APA format to format citations and references free of errors.

Faculty question
If you would prefer to focus on a specific disease state of the thyroid, you can choose the answer the following.
Please, provide the pathophysiology of Hashimoto’s disease. What are common clinical presentations? What labs/screenings would you complete for diagnosis? What are possible treatment options?
Thank you, Dr. Hull

Pathophysiology of PUD
Bayani presents different symptoms including confusion, increased thirst, increased urination, and foul-smelling urine but PUD does not entirely explain the clinical picture. Some of these symptoms such as stomach pain are compatible with PUD but the urinary and metabolic findings necessitate further investigations for other conditions.
Peptic Ulcer Disease, (PUD) develops when there are defects in gastric or duodenal mucosa due to aggressive factors that may include gastric acid and pepsin which overwhelm normal mucosa defenses. PUD major causes include Helicobacter pylori (H-pylori) infection and the use of nonsteroidal anti-inflammatory drugs (NSAIDs). H-pylori has the potential to disrupt the protective mucus barrier and produce chronic inflammation which makes the mucosa more vulnerable to gastric acid (Pertiwi et al. 2026). On the other hand, NSAIDs inhibit cyclooxygenase and reduce prostaglandin production. Prostaglandins help in maintaining mucus and bicarbonate secretion and mucosal blood flow which means that their reduction increases mucosaâ€s susceptibility to injuries. With continued acid and pepsin exposure, mucosal erosion takes place and it then progresses into an ulcer.
PUD has different clinical manifestations including nausea, abdominal pain, early satiety, bloating, and indigestion. In complicated PUD cases, gastrointestinal bleeding is present and is manifested by weaknesses, pallor, anemia, or dizziness. Also, in the case of perforation, it is possible for a patient to have severe abdominal pain and peritoneal findings (Pertiwi et al. 2026). Looking into Bayaniâ€s case, he presents abdominal pain but for its consistency with PUD, it should be related to meals or epigastric issues. Also, if the abdominal pain is related to perforation, the stomach examination should not have the soft and nondistended description.
Bayaniâ€s findings partially support PUD due to the presence of stomach pain. However, several manifestations are not consistent with PUD. For example, his increased thirst (polydipsia), increased urination, (polyuria), and mild confusion raises a concern for a metabolic disorder such as hyperglycemia, particularly diabetes mellitus. Hyperglycemia leads to osmotic diuresis, and this explains increased urination and compensatory thirst (ElSayed et al. 2023). Also, the presence of metabolic abnormalities can lead to confusion in an individual. On the other hand, Bayaniâ€s foul smelling urine raises a concern for urinary tract infection (UTI), but the odor alone does not justify the presence of UTI unless further evaluation is done. In older adults, UTI can lead to acute confusion if there is a present infection. In this case, PUD could explain the abdominal pain, but it does not adequately explain Bayaniâ€s complete presentation.
For PUD, the main diagnostic tests include H. pylori testing using a urea breath test or stool antigen test. A positive result would support H-pylori associated with PUD. Upper gastrointestinal endoscopy (EGD) is also another test which is useful when investigating suspected ulcer disease in older adults or in the presence of alarm features (Pertiwi et al. 2026). This test allows direct visualization of gastric or duodenal ulcers and biopsy for H. pylori or malignancy. CBC is another possible test to identify the presence of anemia and if it turns positive, it could indicate gastrointestinal blood loss. In this case, a bleeding ulcer can lead to reduced hemoglobin and hematocrit. Other tests would include fecal occult blood testing to help identify gastrointestinal bleeding and gastric biopsy to help evaluate H. pylori and exclude malignant lesions when appropriate.
Bayaniâ€s presentations would require further testing to determine the right diagnosis. Urinalysis would be necessary to assess leukocyte esterase, nitrites, white blood cells, bacteria, blood, glucose, and ketones. Urine culture would also be necessary once UTI is suspected since it can identify the causative organism and guide on antibiotic therapy. Serum glucose and HbA1cshould also be obtained since polyuria and polydipsia strongly suggest possible hyperglycemia(American Diabetes Association. (n.d.). The basic metabolic panel analyzes electrolytes, BUN, creatinine, and glucose which can help identify dehydration, renal dysfunction, and metabolic abnormalities that could contribute to confusion. In the case of elevated glucose serum or urine ketones and serum osmolality may be appropriate to evaluate for a hyperglycemic emergency.
When compared to a student assigned diabetes mellitus, the condition may fit several aspects of Bayaniâ€s manifestations better than PUD. For example, Bayani presents polyuria and polydipsia which are classic manifestations of hyperglycemia (American Diabetes Association. (n.d.). Severe hyperglycemia can lead to altered mental status which could explain Bayaniâ€s confusion. Diabetes can also increase susceptibility to urinary tract infections which can explain foul-smelling urine in Bayaniâ€s case.
