Purpose
The purpose of this assignment is to formulate an assessment plan specific to a selected population and cross-sector complex system initiative.
Course Outcomes
This assignment enables the student to meet the following course outcomes:
CO 1: Examine the role of advanced nurse leaders in advocating for systemic changes across complex systems. (POs 1, 3, 4, 5)

Identify the targeted population you have selected.
Describe your selected cross-sector complex system initiative.
Section 2: Suggested Sources
List one source from the Cross-Sector Assessment Plan in Complex Systems Resource Document that may be useful in your assessment.
List at least one additional source from the Cross-Sector Assessment Plan in Complex Systems Resource Document that may be useful in your assessment.
List at least one additional source you found that will help assess the target population.
Provide a rationale for each selected source.
Section 3: Demographic Sources
List at least three specific demographic data points you plan to collect.
Provide a rationale for the selected priority demographic data point.
List each demographic data point’s source (website or point of contact).
List at least one stakeholder you will involve in gathering demographic information.
Provide a rationale for involving the selected stakeholder.
Provide stakeholder connection information (website, email, phone, etc.).
Section 4: Health Indicators and Profile Plan
List at least three health indicators most relevant to collect for your assessment.
Provide a rationale for each selected health indicator.
Provide a source for information (website or point of contact).
List at least two sources for the target population’s health profile (focusing on social determinants of health, lifestyle practices, and cultural values and beliefs).
Provide a rationale for each selected health profile source.
Provide a source for information (website or point of contact).
List at least two assessment findings you will need to collect regarding the impact on the targeted population’s health.
Provide a rationale for each selected assessment finding.
Provide a source for information (website or point of contact).
List at least one stakeholder you will involve in gathering health indicators.

Provide stakeholder connection information (website, email, phone, etc.).
Section 5: Assessment of Subsystems Plan
List at least four subsystems you will need to assess.
Include where you plan to collect the source for each identified subsystem (at least four).
List at least one stakeholder you will involve in gathering primary sources of subsystems.
Provide a rationale for involving the selected stakeholder.
Provide stakeholder connection information (website, email, phone, etc.).
Section 6: Assessment Barriers
Identify at least one potential challenge in accessing specific sources and provide a well-reasoned explanation for their assessment.
Outline at least one clear and effective strategy for verifying the accuracy and reliability of the sources collected.
Identify at least one anticipated barrier to effective communication with stakeholders and community members.
